If you haven't already, please call us at 847-328-1440 and provide us with your name and contact information so we can schedule your first visit and get the necessary paperwork ready.  It will also be helpful to have your pet's previous medical history and records prior to your visit so our doctors will be able to answer whatever questions you may have about their continuing care.  Animal Hospitals often require a client's permission to release information, so if you do not have copies of your pet's records, please contact your previous animal hospital and arrange to have the information faxed to us at 847-328-2095.  We will need vaccine history, surgery/dental reports, results of any testing your pet has had and the names and dosages of all your pet's medications.  This is especially important if your cat or dog has any serious medical concerns or you need refills of medications or prescription foods at the time of your visit.

We recommend arriving five minutes early for your first visit to fill out a brief information form about your pet(s) and so we can copy any paperwork you bring with you.
