New Clients

Thank you for choosing Berglund Animal Hospital to be your partner in your dog's health care!  We look forward to getting to know you and your pets.

If you haven't already, please call us at 847-328-1440 and provide us with your name and contact information so we can schedule your first visit and get the necessary paperwork ready.  It will also be helpful to have your dog's previous medical history and records prior to your visit so our doctors will be able to answer whatever questions you may have about your dog's continuing care.  Animal Hospitals often require a client's permission to release information, so if you do not have copies of your pet's records, please contact your previous animal hospital and arrange to have the information faxed to us at 847-328-2095.  We will need vaccine history, surgery/dental reports, results of any testing your pet has had and the names and dosages of all your pet's medications.  This is especially important if your pet has any serious medical concerns or you need refills of medications or prescription foods at the time of your visit.

We recommend arriving five minutes early for your first visit to fill out a brief information form about your dog(s) and so we can copy any paperwork you bring with you.

New Puppies

Congratulations on your new puppy!  Hopefully you've already taken a lot of pictures because soon your new little wiggling, barking bundle of energy won't be so little.

We recommend that all new puppies be examined by a veterinarian within one week of being picked up from the shelter or breeder to ensure they are in good health.  Breeders and shelters may have their own timetables for returns or covering any illnesses your puppy may develop after purchase/adoption.  Please call our office to get your puppy's information in our computer system so we can get his or her chart ready.  If you have easy access to a fax machine, you can fax to us any paperwork the breeder/shelter gave you.  Our fax number is 847-328-2095.  If you'd rather, feel free to bring the paperwork with you to your puppy's first appointment.

At your first visit, we recommend bringing in a stool sample (as recent as possible) to be checked for parasites.  Your puppy will also receive any vaccines which may be due. This visit is a great opportunity to ask the doctor any questions you may have about puppy care, training or behavior problems.  Our veterinarians will also provide you with information and resources that will help you with future questions you may have.  Any vaccine booster appointments your puppy needs can be scheduled with the receptionist at the end of your visit. New Adult Dogs

Thank you for giving an adult dog a new home!

We suggest that all new dogs be examined by a veterinarian within one week of being picked up from the shelter or breeder to ensure they are in good health.  Breeders and shelters may have their own timetables for returns or covering any illnesses your dog may develop after purchase or adoption.  Please call our office to get your new dog's information in our computer system so we can get his or her chart ready.  If you have easy access to a fax machine or your new dog has an existing medical condition and you need medication/food refills at your first appointment kindly fax to us any paperwork the breeder/shelter gave you.  Our fax number is 847-328-2095.  If you do not have access to a fax machine you can bring the paperwork to your first visit.
